Who we are
Lauren Tabelin Veterinary Physiotherapy (LTVP) provides veterinary physiotherapy to horses and dogs across Hertfordshire. For any question about this policy or your data, contact lauren@vet-physio.co.uk.
What we hold
Your name, contact details and address or yard location; your animal's name, species, age and clinical details; treatment notes, assessments and exercise programmes; photographs and videos taken during sessions to monitor progress; and correspondence with you and your veterinary surgeon.
Why we hold it
To provide physiotherapy treatment, to keep a clinical record of your animal's progress, to arrange appointments and send treatment notes, to liaise with your veterinary surgeon about the case, and to manage payment of fees.
Lawful basis
We process your data to perform our contract with you and on the basis of your consent, which you give when you instruct us to treat your animal. In holding and using data about clients, owners and animals we comply with the General Data Protection Regulation (GDPR) and the Data Protection Act 2018.
Who we share it with
Your veterinary surgeon, occasionally, to discuss progress or provide access to clinical notes. Insurers, where specifically required, including clinical histories and case records relating to you or your animal. Debt collection services, only where accounts remain unpaid. We do not share your information with any other third party without your prior permission.
Client confidentiality
The physiotherapist–client relationship is founded on trust. In normal circumstances we will not discuss or disclose to any third party any information about a client or animal without either express or implied consent.
Photographs and case records
Photos and videos are regularly taken during sessions as a way of monitoring changes in an animal's condition, and are occasionally used on our social media pages. If you would prefer your animal's images are not shared, please tell Lauren. All clinical records remain the property of LTVP; any veterinary surgeon or paraprofessional taking over the case may be forwarded copies of relevant records only on your request or with your consent.
How long we keep it
Clinical records are kept for as long as your animal is under our care and afterwards for as long as is required for professional record-keeping. Contact details are removed when they are no longer needed.
Your rights
You may ask for a copy of the information we hold about you, ask us to correct anything inaccurate, or ask us to delete information we no longer need. To do any of these, email lauren@vet-physio.co.uk.
The client portal
If you create an account in our client portal, your email address is used to match you to your animal's treatment history cards. Only you and LTVP can see those cards. Your account is protected by your own password, and you can ask us to close it at any time.
Cookies
This website uses only the cookies needed to keep you signed in to the client portal and to operate the site. We do not sell or share your data.
